MCB offers eligible employees a choice of three medical and prescription drug insurance coverage options, offered through CareFirst:
• BlueChoice Advantage HSA Option 1 1600
• BlueChoice HMO HSA Option 4 1600
• BlueChoice Advantage Option 3-S 1000
The Advantage plans are national plans that offer in-network and out-of-network benefits. When seeing providers in the local CareFirst service area (Maryland, DC and Northern Virginia), members must utilize the BlueChoice HMO network of providers for in-network benefits. When seeing providers outside of the local CareFirst service area (Maryland, DC and Northern Virginia), members are subject to the Preferred Provider network for in-network benefits. Out-of-network benefits through nonparticipating providers are also available.
The HMO plan is a local plan that offers in-network benefits only. Members must utilize the BlueChoice HMO network of providers for in-network benefits. This network covers providers in Maryland, DC and Northern Virgina.
Both medical plans provide flexibility to seek specialist care without a referral. Prescription coverage is included. Please refer to the medical chart for more detailed benefit information.
MCB offers vision coverage through CareFirst to eligible employees. Employees may go to any provider of their choice; however, when utilizing the Davis Vision network, members receive a higher level of coverage with lower out-of-pocket costs. CareFirst BlueVision Plus offers out-of-network benefits as well.
MCB offers two dental plans through CareFirst to eligible employees. Employees may go to any dentist of their choice; however, when utilizing an in-network dentist, members receive a higher level of coverage and lower out of pocket costs. The CareFirst Preferred Dental plan offers in-network and out-of-network benefits. There is a $1,000 annual maximum benefit amount. Orthodontic benefits are included for dependent children under age 19. The CareFirst BlueDental Plus plan offers in-network and out-of-network benefits. There is a $2,000 annual maximum benefit amount. Orthodontic benefits are included for dependent children and adults.
